Support Coordination

AlignAbility Support Coordinators are here to make understanding and implementing your plan as easy and straightforward as possible.

The job of our Support Coordinators is to take your plan combined with your needs and preferences and use these to connect you to and set you up with a network of support that will come together to help you move towards your goals.

These supports might include support worker services, recovery coaching, allied health services, housing & living options, mainstream & community services and more.

Your support coordinator can also assist you in establishing service agreements, monitoring budgets, liaising with Local Area Coordinators and Planners and preparing for NDIS plan reviews,
